Please read this eStatement Consent Disclosure carefully and keep a copy for your records.

Cresco Capital Inc. (“we”, “us”, or “Cresco”) is required by law to provide to you certain written periodic statements, notices and other communications. Described below are the terms and conditions for receiving those communications electronically from Cresco, as required by the Electronic Signatures in Global and National Commerce Act (“ESIGN”).

Consent to Electronic Delivery of Statements and Other Communications

By selecting the box indicating “I agree with the terms and conditions of this application” you are consenting to receive consumer disclosures electronically, including any adverse actions regarding your application (“Communications”). By consenting to enroll in electronic delivery of eStatements, you are agreeing to electronically receive all of the types of the Communications that Cresco is able to provide in an electronic format. The types of documents available electronically are subject to change, and if additional Communications become available in an electronic format, you agree to receive those Communications electronically. Your consent remains in effect until you give us notice that you are withdrawing it. You also agree that Cresco does not need to provide you with an additional paper (non-electronic) copy of the Communications disclosed herein, unless specifically requested.We may always, in our sole discretion, provide you with any Communication in writing, even if you have chosen to receive it electronically.

Paper Delivery of Disclosures and Notices

You have the right to receive a paper copy of the communications sent electronically. To receive a paper copy, please request it by sending an e-mail message with your name and mailing address to the underwriting manager, at credit@lonemountaintruck.com. Be sure to state that you are requesting a copy of the disclosures or notices. You may not have to pay a fee for the paper copy requested.

Withdrawal of Electronic Acceptance of Disclosures and Notices

To withdraw your consent to receive any future Communications electronically you must send an e-mail message with your name to the underwriting manager, at credit@lonemountaintruck.com. Be sure to state that you are revoking your consent to receive electronic Communications.

Requirements to Access and Retain Information

In order to receive and retain electronic Communications, you must provide, at your own expense, an Internet connected device that is compatible with Cresco’s products. Your device must meet the minimum requirements outlined below. You also confirm that your device will meet these specifications and requirements and will permit you to access and retain the disclosures and notices electronically each time you access and use Cresco’s products. You must also have an active e-mail address. It is your responsibility to provide and maintain current email contact information with us. You can update your email address by sending an e-mail message to the underwriting manager, at credit@lonemountaintruck.com. Be sure to state that you are changing your e-mail address.

Hardware and Software Requirements

In order to access, view, and retain electronic communications from us, you must have:

• An up-to-date Internet or mobile browser to access your email, or if accessing through your mobile device, one of the following mobile operating systems: Apple iOS 6.0 or later, or Android OS 2.3 or later;
• Local, electronic storage capacity to retain our Communications and/or a printer to print them;
• A valid e-mail address and software to access it;
• An up-to-date device or devices (e.g., computer, smartphone, mobile device, tablet, etc.) suitable for connecting to the Internet or for accessing our mobile websites;
• Software that enables you to view files in the Portable Document Format (“PDF”).
